-
Applicant consent form
The checklist says to complete, sign, and upload the Applicant Consent Form after the invitation, once the application in the e-filing portal has been reviewed. A representative may not sign it.
-

The checklist says you must upload a copy of the personal details page of your passport and every page that shows a Canadian visa or entry stamp.
-
Education documents
The checklist splits education by path. A recent Ontario graduate, a Canadian credential, and a foreign credential each have their own documents. Studies outside Canada need an educational credential assessment, and the issuing organization must be authorized to share the result. The accepted documents for each path stay on the checklist.
-
Language test results
The checklist says you must upload English or French language test results taken within the two years before the application. The accepted tests stay on the checklist.
-
Employment reference letters

-
Proof of compensation
The checklist says you must upload proof of compensation for the work experience included in the application, such as pay slips or bank statements for the first and last month, income tax documents, or an employer letter explaining why those records are unavailable.
-
Resumé
The checklist says you must upload your most current resumé, covering work and education, the duties of each position, and your current residential address, email, and phone number.
-
Intention to live and work in Ontario
The checklist says you must list established ties to Ontario. Examples on the page include Ontario work, study, family, and housing.
-
Expression of interest scoring-factor documents
The checklist says scoring factors are separate from the stream criteria, and that you and your employer must upload the documents that support the points claimed. The factor list, including any earnings threshold, stays on the checklist.
If this also applies
-
If a spouse, common-law partner, or dependent child is included, also add each dependent family member passport personal details page.
The checklist says that if family members are included, you must upload the personal details page from the passport of each dependent family member. That can include a spouse or common-law partner, children under 22, and their children.
-
If you are in Canada, or you have been, also add current and previous status documents.
The checklist says to upload current and previous status documents from Immigration, Refugees and Citizenship Canada or the Canada Border Services Agency, such as a work permit, study permit, temporary resident visa, or other Canadian immigration document. Maintained status uses the acknowledgement letter, fee receipt, or mailing receipt the page names.
-
If the Ontario occupation requires a licence or other authorization, also add that licence or authorization.
The checklist says that if the job offer is in an occupation that requires a mandatory licence or other authorization in Ontario, upload a copy of that licence or authorization.

If you already work for the employer in the job-offer position, also add your two most recent pay slips.
The checklist says that if you currently work for the employer in the same position as the job offer, include copies of your two most recent pay slips.
-
If you have them, you may also add work contracts for the experience on the application.
The checklist lists work contracts as optional support for the work experience included in the application.
-
If the work experience was in Canada, also add T4 statements and notices of assessment, or an explanation.
The checklist says that if the work experience was in Canada, upload the T4 statements and Canada Revenue Agency notices of assessment for the relevant years, or an explanation if you do not have them.

Only add other work documents when the checklist asks for them.
The checklist says other relevant work documents may be included. It also says not to add non-mandatory documents that the page does not ask for.
-
If you are applying as a self-employed physician, also add the medical licence.
The checklist says a self-employed physician’s medical licence is the document that supports the occupation points, and that work-history documents do not apply to a physician who can apply without a job offer.
-
If you are applying as a self-employed physician, also add proof of the OHIP billing number.
The checklist says a self-employed physician must provide proof of OHIP billing number registration that shows a practice address when location points are claimed.
